Start with Outlander (1991), where WWII nurse Claire Randall touches a standing stone at Craigh na Dun and falls back to 1743 Scotland, and simply move forward through the nine main novels. There is no prequel to read first and no recommended shuffle.
The one real complication is the spin-off material. Gabaldon also writes the Lord John Grey series, following a recurring supporting character, plus a growing shelf of novellas and short fiction (collected in things like "Seven Stones to Stand or Fall"). These slot into the chronology between the big books, and some readers love weaving them in for extra context on Lord John, Roger, and others. But they are genuinely optional: you can finish the main saga without ever touching them and miss nothing essential to Claire and Jamie's story. Treat them as side quests, not required reading.
